Palliative RN Clinical Navigator, M-F, 8:00AM-4:30PM

Position Summary: The Palliative RN provides episodic advanced serious disease management, urgent visits, promotes preventative care and wellness, pain management, and augments the provision of end of life/palliative care. Key responsibilities include assessing patient needs, coordinating consults, providing education, liaising with various care providers and community resources, and maintaining accurate documentation.

Compensation based on experience: $93,600 - $97,760 per year

Experience:

• Minimum of one year in an acute care setting or comparable work experience required.
• Minimum of one year oncology and home care experience preferred.
• Minimum of one year hospice or palliative experience required.
