LPN Field Staff Nurse, 20 hours per week - flexible hours available, M-F

Position Summary: The Field Staff Nurse is responsible for opening new cases, creating care plans and supervising all caregivers in the client home setting.

Functions & Responsibilities:

• Provides directions to caregivers upon initial visit and creates care plans
• Provides supervision during placement of a caregiver on vacation coverage
• Communication with families of clients as necessary, optimizing communication between clients, family, caregiver and Private Pay staff
• Proficient in planting and reading Mantoux/PPD tests
• Provide outstanding customer service to all clients, potential clients, staff and providers in the community
• Ability to assist with answering incoming calls, taking intakes

Skills and Knowledge:

• Strong clinical knowledge and understanding of home care practices
• Strong interpersonal and communication skills
• Strong organizational skills with the ability to set priorities and establish goals

Education:

• Active Licensed Practical Nurse in Massachusetts
• Graduate of a professional nursing program
• Computer literacy required

Experience Preferred:

• 2-4 years of nursing experience preferred
