VNA Care is seeking a PT for our Acton/Concord team.

Position Summary: Provides therapeutic care to patients and families that focuses on restoring, promoting or maintaining function and minimizing the effects of illness or disability.

Schedule: Monday- Friday, 8am-4:30pm

Visits available in and around: Acton, Concord, Carlise

Training: We offer an extensive Home Health orientation program. Work with a preceptor, in the field, for 6-8 weeks and have the support of an Educator while learning Case Management, Oasis E documenting, Time Management and EMR system; Home Care Home Base. VNA Care believes in an ongoing supportive work environment.

Requirements:

• Compassionate, enjoy working with patients and their families, organized and ability to multi-task.
• Good standing MA PT License
• Minimum of 1 year PT experience
• Willingness to travel within a territory
